Our integrated care model delivers not just health services but a lifeline of education, access to healthcare and essentials to those most in need.

As we reflect on the year ahead, we’ve set an ambitious goal to continue carrying out our mission in 2024 and beyond.

With your year-end gift, you’ll enable us to provide patients with primary healthcare, women’s wellness services, HIV testing and counseling, and affordable medication through our pharmacy—and make our program and services accessible through telehealth visits and our Good Health Shuttle, which makes 100+ trips a day and provides patients with safe and reliable round-trip transportation, free of charge.

Your support will allow us to continue running vital education programs in our clinic, homeless shelters and Department of Education classrooms. Our teaching kitchen can go on helping families experiencing homelessness learn and practice healthy eating and cooking through demonstrations, workshops and tastings. Camp Rise Up can continue to rise to the challenge of helping children cultivate essential life skills such a s healthy relationship building and financial literacy.
