What happens at the visit
Whole-child checkup: height, weight, vitals, development and age-based screenings
Immunizations: given at the same visit when due, and based on your child’s record
Vision & hearing screenings: both essential to learning and success
Chronic condition care: asthma, diabetes, and behavioral health referrals as needed
Forms: school health, sports clearance medication permission and asthma action plans will be completed in five business days after your appointment
If you don’t have your child’s records, bring what you can. We’ll help track down previous vaccines and coordinate next steps.

Insurance card (if you have one) and photo ID
No insurance? No problem—we’ll still see you.

Tips for a healthy school year
Smart habits that can make a big difference for attendance, learning and well-being
Daily routines
Sleep: Keep a steady schedule. Most school-age kids do best with 9-12 hours; teens with 8-10 hours.
Breakfast and snacks: Aim for protein + fruit and whole grains. Pack a refillable water bottle.
Move every day: About 60 minutes of active play or sports will keep your student in tip-top shape.
Say “no” to germs
Handwashing: Wash (for at least 20 seconds) before eating, after using the bathroom and after recess. Use sanitizer if there is no access to running water.
Cover coughs and sneezes with your elbow or a tissue. Make sure you throw out your tissue and wash your hands afterwards.
Don’t share water bottles, lip balm, cosmetics or earbuds.
Vaccines and preventive care
Stay up-to-date on immunizations and get a flu shot each fall.
Vision and hearing checks, and dental cleanings boost learning.
Make sure your child is healthy enough to participate in sports and extracurricular activities. Check with your family clinician.
Managing ongoing conditions
Send the school your child’s action plans (asthma, allergies, diabetes)
Make sure medications/inhalers/epinephrine are in school with signed forms
Update emergency contacts and preferred pharmacy
When to keep your child home
If they have a fever higher than 100.4˚F, are vomiting or have diarrhea in the past 24 hours, have a severe cough, pink eye symptoms or a new or unexplained rash.
Your child can return when fever-free for 24 hours without medication and symptoms are improving. Call us if you’re unsure.
Mental health and safety
Do check-ins about stress, friends, social media and sleep
If you see persistent changes in mood, appetite and grades, reach out to us—we can help.
Review bullying and online safety. Set screen time and a device-free bedtime.
Encourage eye breaks during screen time and homework.
